Advance Care Planning

Research indicates that, as of 2005, fewer than 30% of Americans have an advance directive.  Healthcare providers, attorneys, social service and aging organizations, faith communities, employers and families are starting to provide advance directive information/education to their constituents.  Caring Connections has developed resources to help communities explore advance care planning and promote the importance of expressing wishes for end-of-life care before a crisis.
