An advance directive is a legally binding document that gives instructions for your healthcare in the event that you are no longer able to make or communicate those decisions yourself. - Due to formatting and printing requirements, for some states a blank page will appear in the Acrobat Reader as the first page. This does not mean the document has not loaded correctly.

- Once completed, save and print the form, or store it digitally. If using paper, you should scan the form and save it everywhere you can – including your phone – and distribute them to your inner circle, your physician, and anyone else you think should have it.
What happens if you do not have an Advance Directive in Iowa and are unable to speak for yourself
The decisionmaker is decided by order of priority:
- A spouse
- An adult child or, if more than one, a majority reasonably available for consultation
- A parent, or both if reasonably available
- An adult sibling
